Welcome to the Torquay PCN

Working together to help keep patients happy, healthy, and well.

The Torquay Primary Care Network is a network of 4 practices, Brunel Medical Practice, Chelston Hall Surgery, Croft Hall Medical Practice and Southover Medical Practice, all with a goal of improving patients health and wellbeing. The PCN is led by a Clinical Director and made up of a team of Advance Nurse Practitioners, Pharmacists, Physician Associates, Health and Wellbeing Coaches, Care Coordinators, and other clinical profession working within general practice.

All four practices work together to ensure that every patient within the Torquay Primary Care Network can benefit and take advantage of the high-quality health and wellbeing services when needed.

As a patient you are still registered with your existing practice however, you do get to benefit from all 4 surgeries working on services together and becoming more resilient.

What is a Primary Care Network?

A primary care network is a group of practices who work together, and the focus is local patient care. They have been around since 2019 and most practices across England are now working as part of one. Due to the increasing demand and shortage of clinical staff, practices have found that they can provide a more resilient, beneficial, and personalised care for patients when working together with other surgeries.
